tl;dr: Ultimate fix for MGS2 PC. Play and/or help me to improve it. Download link on my GitHub page (you can find it at the bottom). Enjoy it!
I'm a big fan of the MGS saga. I've started from MGS1 and I simply loved it. Probably my favourite PSX game! After that, I've played the second chapter a lot in my youth days. I was maybe too young to understand fully the plot (hell: Even today probably is too convoluted) but I simply loved it. After that, I never played it again. I've re-played all the saga (even the spin-off and Ac!d) LOT of times but I've never put again the Substance DVD in my xbox/pc.
Probably, after all this times, MGS2 Substance for PC is the WORST console conversion ever made. Graphic it's broken, control are broken, don't work lot well on modern system. The pc-setupper is a simply .exe patching, just like a strange trainer that inject settings inside the game. Setup the controller in a good configuration is like to paint the mona lisa blindfolded cause it's impossible to bind all key to the gamepad. Even trigger doesn't work. Vibration doesn't work. Snake look fat like the pig-chainsaw from Dead Rising 2 cause it's stretched on 16:9 monitor. Konami point score is offline. Player need to download cracked exe only to see shadow and water cause .exe need to be patched for every different brand of gpu adapter.
After all this year, this HAS come to an end. Konami is too busy to buy gyms and pachinko (?) to release a good HD edition on pc (and repeat the same things for silent hill saga, that work FLAWLESSLY on HD monitor with amateur patch). Some guy has released a 1080p version of MGS Integral. I made the next step:

How it work, in a sintetic way:

Overwrite MGS2SSetup.exe inside the 'GAME DIRECTORY/BIN' folder

Very very VERY VERY important things that you have to know!:
I've tried it only on the EUROPEAN VERSION of the game. Please let me know if it work on American or Japanese one! (Onestly I dubt that work on the last one, but who knows...)
Laptop mode is required if you play on a laptop that have dual gpu system (Intel + dedicated). Better enable if you are on a laptop (even with a single gpu!)
Widescreen Fix (Mantein Aspect Ratio Option) work only on 16:9 or 16:10 resolution. If, like me, you have some 'strange' aspect ratio resolution (like 1366x768 on my laptop) stick on 1280x720 or 1920*1080, it would look good anyway. It's a
bugfeatures by the custom fix made by nemesis2000. Probably together someday we will fix it.Since the V's Fix open, delete, create, see, copy, do and redo lot of things it can BE blocked by Windows UAC. Isn't doing any harmful things! My word! (If you note it even the original launcher has the same 'problem'). You can be warned for opening 'VOX.exe' (that is done by Konami and it's the original 2.0 patch) and 'MGS2SSetup.exe' itself. Don't worry and go ahead. If you don't want to be assilled by windows anymore right click the file -> properties -> compatibility -> always open with admin right.
.net 4.5 required It's default installed on Win8/8.1 and Win10. On Win7 you need to install it by yourself, but It is likely you have already installed it for some other reason. (NO XP SUPPORTED. Upgrade your damned pc)
**Any advice? things to add? BUG???? Tell me as soon as you can! Feedback it's important cause I didn't tested the fix on your pc, so if there's any things out of place I want to know it to improve the fix as much as I can.
